- Original and a copy of the identification card (residence permit for foreigners)

- Health report (From state hospitals or private health organisations)
- Blood group document

- Expenditure receipt from the Ministry of Finance (Shall be paid according to the driving license class)

Re-Issuing of Lost Driving Licenses
- A petition addressed to Traffic Registration Department
- Original and a copy of the identification card
- If not written on the lost identification card, blood group document
- One photograph
- TR-SF-I from and Annex-30 card from Drivers Association
- Driving license fee 18.500.000.-TL
Changing of Driving Licenses due to Wear & Tear or Change in Identity Card
- A petition addressed to Traffic Registration Department
- Original and a copy of the identification card
- If not written on the lost identification card, blood group document
- TR-SF-I from and Annex-30 card from Drivers Association
- One photograph
- Former driving license
- If there is change on the identification card, the document indicating such change (Marriage certificate or photocopy of written divorce decision)
- Driving license fee 18.500.000.-TL
